Pelvic floor muscle should be 0 position

The term “zero position” refers to a posture or stance that allows for quick and efficient movement from a static position to dynamic motion. This is especially useful to prevent unnecessary movements, react promptly to changes, and avoid injury by returning to a baseline stance.

For the pelvic floor muscles, the “zero position” is a posture that allows a person to instantly shift to a “protective stance” when abdominal pressure is applied. To check if someone is in the pelvic floor zero position, we use a “perineal ultrasound.” This method helps confirm if the position easily allows the formation of a protective “arch,” making it understandable for anyone to assess.

Today, we had a patient with postpartum pelvic organ prolapse and urinary incontinence. Although she was able to contract her pelvic floor muscles relatively well (albeit with compensatory movements), we were able to establish a shared understanding of the zero position and set a target for her treatment.
